Create survey within comment box

I am looking for a way to create a separate comment tool that, when called, opens with a brief survey that can be added to a point within the pdf file. I'd like to use this for basic assessments of buildings where the data is very repetitive. For example, if I am looking at 300 windows, I would be able to add a survey to the tool such as "Broken? (yes/no)," "# of broken windows? (_)," etc., and then be able to call the tool multiple times with the survey already embedded in the comment box. Any help/suggestions would be appreciated. Of course, it doesn't necessarily matter if this is a true "comment" box, but it will require user-entered data. Thank you!
