How to embed videos and images into a dynamically created pdf file?

Sir How to generate dynamic pdf forms using livecycle and how to manipulate the file with videos,images and other text including signature,notes,comments etc?I created static pdf file and route it according to the process.But i need dynamic form.The form contains fields that are manipulated from database.
